GOOD FAITH ESTIMATE- The Mother Road Dietitian
In accordance with the Good Faith Estimate Law, when requested by the clients we are required to provide you with an estimate of the total price in which you may be charged for nutrition therapy services. Estimates are not binding on the practitioner. The final price may vary from the estimate based on the patient’s nutrition therapy needs. The estimate is only good for 30 days.
No Surprises Act of 2022
In accordance with the No Surprises Act of 2022, it is the responsibility of the client to see participating providers within your health insurance plan. Should your plan not be participating with The Mother Road Dietitian, LLC or a provider of The Mother Road Dietitian and you receive nutrition therapy services, you will be billed the Out of Network patient responsibility or non-participating balances as they pertain to services provided.
